E3D Volcano adapter for FT-5

Volcano adapter for FT-5.

This design allows you to use the same screws used to hold original fan down.

I have used this on 30 hr prints with no problem

Intentional 1.5 mm gap left between part to allow clamping action on E3D heat sink.
